This position will be located based on the selected candidate's area of residence and will be expected to travel to the following offices in the counties of: Chatham, Toombs, Ware, Lowndes and surrounding areas.

Job Description
Pay Grade: I
Under broad supervision, provides assistance to victims in the alleviation of financial, emotional, and physical consequences of victimization. Provides information and referral services and assistance with outreach and public awareness activities. May serve as lead worker.
Role And Responsibilities
- Provides guardianship case management services in accordance with guardianship law, the probate court order and letters establishing guardianship, and Department policies and procedures.
- Arranges for the support, care, education, health, and welfare of the guardianship client by developing care plans that provide for the coordination, implementation, and monitoring of services and/or supplies for the guardianship client.
- Practices surrogate decision-making for the guardianship client that reflects least restrictive alternatives and taking in account the person’s expressed desires and beliefs.
- Considers the needs, wishes and resources available to the guardianship client when providing guardianship case management services.
- Advocates on behalf of the guardianship client.
- Works cooperatively with the guardianship client’s trustee, conservator, representative payee, or other fiduciary individual to coordinate a plan to ensure the person’s financial needs are met.
- Provides information and education to guardianship clients, families, providers, agencies, professionals, and other stakeholders in the community in the assigned multi-county area.
- Documents case management actions in the case record as required by Department policy.
- Acts as a mandated reporter of cases of abuse, neglect, or exploitation and makes other reports as required by law, policy or procedure.
- Performs other professional responsibilities as assigned.
In addition to the duties above, the Guardianship Lead Worker will:
- Performs administrative duties at the direction of the supervisor.
- Provides consultation on cases and assists staff in resolving problems encountered in work assignments and provides needed assistance with decision making and policy questions.
- Reads case records for peer review and quality assurance purposes and provide input into the performance evaluation process.
- Provides on-the-job training to staff and serves as a mentor to fellow workers.
- In the absence of the Supervisor, plans and organizes the unit’s work to meet the organization’s objectives. Goals and objectives are consistent with the organization’s mission.
- Delegates and/or assigns and prioritizes work in keeping with employee skill levels, workloads, and completion timeframes.
- Approves leave for other workers, in consultation with District Mgr.
- Communicates organization’s goals and objectives to staff.
- Monitors progress and productivity of assigned staff.
- Evaluates programs, procedures, and processes on an interim basis, as designated by the supervisor.
- Serves as the primary Community Outreach/Education liaison for the unit.
- Makes presentations as requested on Guardianship’s role, aggregate data specific to the community for organizations such as Rotary Club, Kiwanis Club, churches, etc.
- Serves on multi-disciplinary teams.

Minimum Qualifications
- Associate degree in a related field from an accredited college or university AND four (4) years of job-related experience;
OR
- Two (2) years of experience at the lower level Advocate 2 (SSP081) or position equivalent.
Preferred Skills/Qualifications: Preference will be given to candidates who, in addition to meeting the minimum qualifications, demonstrate some or all the following education/skills/experience:
- Two years experience as a Guardianship or APS Case Manager
- Experience working with a specific issue facing client populations such as family violence, crisis intervention, substance abuse, services to the elderly disables.
Note: An equivalent combination of knowledge, education, job or intern experience, training, or certifications that provides the necessary knowledge and skills to successfully perform the job at the level listed may be substituted year-over-year.
